You don’t need insurance to get healthcare at the Great Salt Plains Health Centers.

We offer preventive and primary health and dental care, as well as counseling and our services are available to anyone with insurance or to anyone without insurance on an income-based sliding fee scale.

We offer a mix of medical, dental and behavioral healthcare. All of our locations provide primary medical care, counseling and lab services. We also offer a free 24-hour, 365 days a year, nurse hotline for health questions for our patients.

What to Bring to Your Appointment

When you come in for your appointment, be sure to bring a valid picture ID (driver’s license, state or federal issued ID, etc.) and your appointment fee or insurance co-pay. If you plan to apply for our sliding fee scale, bring your income tax return from the previous year or other proof of income. Also, in the event your current address differs from the address on your ID, please bring a utility bill displaying your new address. Don’t forget to let us know if your address, phone number, employment or family status changes.
